Danbury High School Based Health Center (SBHC) is the first SBHC to open in the Danbury School District, 1994.  It is a licensed clinic that provides medical and mental health services to students who have a SBHC Permission Form on file.  A parent or guardian must complete a consent which gives the staff permission to work with your child.  It is open from 7:30 am to 2:30 pm when school is in session.  We do not replace your medical providers but strive to keep students healthy, in school, and successful by providing on-site medical and mental health services.  All insurances are accepted and we do not charge any co-pay. We also accept any uninsured students while at Danbury High School, REACH, or ACE. An added benefit is that parents can often remain at work while their child is being seen at the SBHC. Permission Form is available in English, Spanish and Portuguese.  Please fill it out in ink and sign at the bottom of BOTH pages.  Complete the history on the back page.  See Privacy Notice.
